If a physical is needed, please schedule ASAP since students will not be allowed to practice if this form is not on file.

Once registered you will receive an email within a few days with any paperwork needed.  IMPACT concussion baseline tests for HS athletes will be administered by the athletic trainers at the High School. MS athletes will have theirs on the first day of practice administered by Dawn Peer. 

**Students should come to the HS training room at the high school and an athletic trainer will meet the group to begin testing.

Athletes are required to complete this once every school year before participating in athletics. If you are in a Fall sport you will not need this. 

 
 
   

Please read and follow these steps carefully to be fully registered.  IMPORTANT:  You are only registered once ALL steps are completed.  You will only sign ONCE during the entire process and that will suffice for ALL documents required. 

 

 
 

Step 1: All students who plan to participate in athletics, cheer or dance must have a physical examination every other school year.  Physical examinations must be performed by a licensed physician, physician's assistant or advanced nurse practitioner.  Athletes WILL NOT be allowed to participate without having a physical exam in good standing.  You only need to complete this once a school year. 

A physical examination taken April 1 and thereafter is valid for the following two school years.  Physical examination taken before April 1 is valid only for remainder of that school year and the following year. 

For the 2024-25 school year if your last physical was prior to April 1, 2023, you need a physical. 

An Alternate Year Card is needed if the Physical was between April 1, 2023 and March 31, 2024.

All physical and alternate year card documentation should be submitted online using the REGISTRATION menu above or email directly to Laura Bignell. We would prefer you submit it electronically but will also accept a paper copy.  An upload link is also available under the registration menu above.

Step 2:   Read all information carefully. Your annual signature will indicate that you have read this and agreed to abide by what is stated in these documents.

   
 
Step 3:  Go the Registration link above and fill out and submit the Online Eligibility Forms and Registration Form    
   
 

Step 4:  Finally, pay your Activity/Athletic fee electronically.  There is a $390 cap per family (can combine middle and high school students)  There is a $195 cap per individual. 

 

 

Once you complete these steps, you are registered and can participate in Somerset activities/athletics.
